The supplement aisle is overwhelming. Here's what science actually supports for menopause, what's marketing magic, and how I decide what's worth trying.

Supplements to Research (and Sara's Sane Approach)
Walk into any health food store and you'll find an entire aisle promising to "balance your hormones," "eliminate hot flashes," and "restore your vitality" during menopause.
The reality? Most of these claims are... optimistic. Some supplements have decent research behind them. Many don't. And a few could actually interact badly with medications or conditions.
So how do you navigate this? Here's my sane approach, complete with what the research actually says.
The Ground Rules: Sara's Supplement Philosophy
Before we dive in, here are my guiding principles:
1. Supplements ≠ Substitutes
They're called supplements because they SUPPLEMENT a healthy lifestyle. No pill fixes a crappy diet, terrible sleep, or no exercise.
2. "Natural" ≠ "Safe"
Arsenic is natural. So is poison ivy. Natural just means "from nature"—it says nothing about safety or efficacy.
3. Quality Varies Wildly
Supplements aren't regulated like medications. Two products with the same ingredient can have vastly different quality. Third-party testing (USP, NSF, ConsumerLab) matters.
4. What Works for Her Might Not Work for You
Individual response varies. Just because your friend swears by something doesn't mean it'll help you.
5. Always Tell Your Doctor
Even "natural" supplements can interact with medications or conditions. Your healthcare provider needs to know what you're taking.
Tier 1: Decent Research Support
These supplements have at least some quality studies showing potential benefit for menopause symptoms.
Black Cohosh
What it is: Extract from a North American plant root.
What research shows:
- Some studies show modest improvement in hot flashes
- Results are inconsistent across studies
- Mechanism of action unclear (doesn't seem to affect estrogen levels)
My take: Worth trying if hot flashes are your main concern. Give it 8-12 weeks.
Concerns:
- Rare liver toxicity reports (stop if you notice yellowing skin, dark urine)
- Shouldn't use with liver disease
- Avoid with hormone-sensitive conditions (breast cancer history) until more is known
Dose often studied: 20-40mg daily of standardized extract
Soy Isoflavones
What they are: Phytoestrogens found in soy products.
What research shows:
- Modest reduction in hot flash frequency and severity
- Better results in some studies than others
- Work better for some women than others
My take: If you tolerate soy well, dietary sources (edamame, tofu, tempeh) are my first choice. Supplements are an option if you don't eat soy foods.
Concerns:
- Controversial with hormone-sensitive cancers—talk to your oncologist
- Can affect thyroid function (especially with existing thyroid issues)
Dose often studied: 40-80mg isoflavones daily
Vitamin D
What it is: A fat-soluble vitamin, really more like a hormone.
What research shows:
- Deficiency is common, especially in menopause
- Important for bone health (critical during menopause when bone loss accelerates)
- May support mood and immune function
- Doesn't specifically treat hot flashes but supports overall health
My take: Get your levels tested. If deficient, definitely supplement. Even if adequate, a moderate supplement may be prudent.
Concerns:
- Can be toxic in high doses (more isn't better)
- Take with fat for absorption
Dose: Depends on your levels. Many need 1000-2000 IU daily, some need more.
Magnesium
What it is: Essential mineral, involved in 300+ enzyme reactions.
What research shows:
- May help with sleep quality
- May reduce anxiety and stress
- Some evidence for hot flash reduction
- Commonly deficient in Western diets
My take: One of the supplements I personally take. Sleep improvement has been noticeable.
Concerns:
- Can cause digestive upset (start low)
- Different forms absorb differently (glycinate or citrate are gentler)
- High doses can cause loose stools
Dose often used: 200-400mg daily
Omega-3 Fatty Acids (Fish Oil)
What they are: Essential fatty acids from fatty fish (or algae).
What research shows:
- Anti-inflammatory effects
- Some studies show modest hot flash reduction
- May support heart health (important post-menopause)
- May support mood and brain health
My take: Good for overall health. Benefits specific to menopause are modest but not nothing.
Concerns:
- Can thin blood (talk to doctor if on blood thinners)
- Quality matters (look for third-party tested, stored in fridge)
- Burp factor is real with cheap brands
Dose often studied: 1-2g EPA+DHA daily
Tier 2: Some Evidence, Less Robust
These have some promising research but need more/better studies.
Maca Root
What it is: Peruvian root vegetable used traditionally for energy and vitality.
What research shows:
- A few small studies suggest possible benefits for mood and energy
- May help with sexual function
- Limited evidence for hot flashes
My take: I tried it. Didn't notice dramatic changes, but some women swear by it. Low risk to try.
Dose often studied: 1.5-3g daily
Red Clover
What it is: Another source of phytoestrogens (isoflavones).
What research shows:
- Mixed results for hot flashes
- Similar profile to soy isoflavones
My take: Similar considerations as soy. Not my first choice, but reasonable to try.
Concerns: Same hormone-sensitive condition cautions as soy.
Valerian Root
What it is: Herb traditionally used for sleep and anxiety.
What research shows:
- Some evidence for sleep improvement
- May take weeks to notice effects
- Mixed results in studies
My take: Worth trying for sleep issues. Smells terrible, but capsules solve that.
Concerns: Can interact with sedative medications.
Evening Primrose Oil
What it is: Source of gamma-linolenic acid (GLA), an omega-6 fatty acid.
What research shows:
- Traditionally used for hot flashes, but research doesn't strongly support this
- May help with breast tenderness
My take: Research hasn't impressed me for hot flashes specifically.
Tier 3: Little Evidence, Lots of Hype
These are commonly marketed for menopause but lack solid research support.
DHEA
What it is: Hormone precursor.
What research shows:
- Limited evidence for menopause symptoms
- Can convert to testosterone and estrogen in the body
My take: This is essentially a hormone, not a "supplement." Should only be used under medical supervision with hormone level testing.
Wild Yam Cream
What it is: Marketed as "natural progesterone."
What research shows:
- Your body cannot convert wild yam's compounds into progesterone
- Claims are not supported by research
My take: Marketing, not medicine. Skip it.
Dong Quai
What it is: Traditional Chinese herb.
What research shows:
- Studies haven't shown benefit for hot flashes
- Traditionally used in combination formulas, not alone
My take: Evidence doesn't support using it alone for menopause.
Various "Hormone Balancing" Blends
What they are: Proprietary combinations of herbs.
What research shows:
- Usually no research on the specific combination
- Hard to know what's actually in them or in what doses
My take: I prefer to try individual ingredients so I know what's working (or not).
Sara's Personal Supplement Stack
For full transparency, here's what I currently take:
Daily:
- Vitamin D3: 2000 IU (my levels were borderline)
- Magnesium glycinate: 300mg at night
- Omega-3: 1g EPA+DHA
- Calcium: 500mg (food + supplement, since I don't eat much dairy)
Tried and stopped:
- Black cohosh: Didn't notice significant difference after 12 weeks
- Evening primrose oil: Didn't help my symptoms
- Maca: Didn't notice much
My approach: Start one new supplement at a time. Give it 8-12 weeks. Track if anything changes. If not, stop and move on.
How to Evaluate a Supplement
Questions to Ask:

Red Flags to Watch For
🚩 "Cures" menopause — Nothing cures a natural life transition.
🚩 Before/after photos — Often misleading or fake.
🚩 "Doctors don't want you to know" — Conspiracy marketing.
🚩 Proprietary blends — Can't tell actual doses of ingredients.
🚩 Only available from one website — Quality supplements are widely available.
🚩 Dramatically cheaper than comparable products — Quality control costs money.
🚩 Wild claims with no citations — Real evidence gets cited.
The Bottom Line
Some supplements may help some women with some menopause symptoms. That's a lot of qualifiers—and that's honest.
The goal is to be a savvy consumer:
- Know what you're trying to address
- Look at actual evidence, not marketing
- Choose quality products
- Give things adequate time to work
- Track your response
- Tell your healthcare provider
And remember: supplements are one tool, not the whole toolbox. Sleep, nutrition, movement, stress management, and potentially medical treatments (HRT and others) all deserve consideration too.
